To take any decision regarding the insurance payment, the below indicated documents have to be submitted:
 

• Application form should be filled by policy holder about the insurance claim
• ID Certificate of policy holder
• National Passport
• The original of the insurance contract (certificate)
• Documents, certifying the medical treatments and stay in medical institutions (officially registered copies);
• Documents confirming medical and other expenses, related to the treatment, provided that the value of all rendered services have to be indicated accurately and each separately (originals of invoices, payment order, receipts)

Depending on circumstances, the additional documents may be required.
